Article 6 — Licensing Procedures

Sec. 3-15.6.01. - Submission of application.

(a)

Any person or agency desiring a license to provide ambulance service in the County shall submit an application to the local EMS agency (on forms provided by the EMS agency). The application must be completed in its entirety and shall be verified under penalty of perjury.

(b)

The local EMS agency shall review all applications for provider licenses and conduct such investigations as it deems necessary to verify the information pertinent to such application and desired license. The local EMS agency shall then prepare a report for the Board as to the request for license, verification of application information, and its recommendation for acceptance or rejection of such application.

(c)

If an application for ambulance provider license is approved by the Board, the County shall issue an ambulance provider license, which license shall set forth the class or classes of service to be provided. Such license shall be valid for a period of no greater than twenty-four (24) months. Nothing in this Chapter, however, shall require the Board to issue such license.
