Follow the table/procedures in IRM 3.12.21.10.2(4) above to identify the originator (to whom the Form 4227 (and document) should be sent for research and correction).
Field 41-B, Total Credit Reduction, is computed using the sum of Credit Reduction Wages in Sections 41-46, multiplied by the percentage shown for each state in Exhibit 3.12.21-9. This field will underprint if a math error exists. See Figure 3.12.21-29.

Prepare Form 4227 with the notation, "Credit Reduction amount incorrect" .
