The Original Assessment Transaction Code is found in Item G (Section 01), Page 1, Form 3413.
If the document is correct as transcribed and a valid transaction code can't be determined, reject the document with Action Code 3.
Attach Form 4227, Intra-SC Reject or Routing Slip, and notate "01-G (Original Assessment Transaction Code) invalid" .
