1-14-1: CERTIFICATE REQUIRED:¶
Every person shall be required to file with the City a certificate from an insurance carrier authorized to do business in the State certifying that the applicant is covered under the provisions of the Workman's Compensation Act of the State, or in lieu thereof a Certificate of Consent to Self-Insurance, approved by the Insurance Commissioner of the State and under the provisions of the Industrial Accident Laws of the State prior to the issuance by the City of any permit to do any building, plumbing, or electrical work. 1-14-2: EXEMPTIONS:¶
An applicant may be exempted from the requirements of Section 1-14-1 by filing an affidavit stating reason for exemption as follows:
(A) Filing an affidavit that applicant is a licensed contractor, and that he will not employ any labor in the prosecution of the proposed work.
(B) Filing an affidavit that the applicant is an owner-builder and that all work will be done by self or immediate family.
(C) Filing a affidavit stating that the applicant will complete the construction of the proposed work or improvement in less than ten (10) days from the time of commencement of the proposed work or improvement, and that the total cost thereof of all labor is less than one hundred dollars ($100.00).
(D) Filing an affidavit stating that applicant is the authorized agent of a church or fraternal organization whose members are donating their time and labor, and are not receiving any compensation therefore, and that none of the work is being donated by a contractor in his capacity as a contractor, and that the construction of said building or improvement is a normal function of the church or fraternal organization.
