Prepare Form 3244-A for each tax period and class of tax involved for which payment is received:

Enter the employer identification number. If there is no EIN on file, see IRM 4.70.13.4.

Enter the form number and MFT.

Enter the tax period in YYYYMM format.

Enter the date the payment was received.

Enter the name and address of the taxpayer.

Enter the check number and the amount in the remarks field.

Enter the amount of the payment in the TC 610 field.

Enter the same amount in the total payment field.

Enter your name and office symbols.
