Siskiyou County Municipal Code Art. 8 Transfer

No provider license issued pursuant to this Chapter shall be transferred through sale, assignment, subcontract, change of ownership (for publicly traded corporations, greater than a five percent (5%) change within six (6) months), reorganization, merger, by operation of law, or otherwise, except upon prior written approval by the Board, and the issuance of a new provider license. Any transfer in violation of these provisions shall result in the provider license being null and void.
