Sec. 3-15.7.01. - Renewal.

An ambulance provider license may be renewed by completing the following requirements:

At least sixty (60) days and no more than ninety (90) days prior to the expiration of a valid license, the ambulance provider agency shall submit a renewal application to the local EMS agency on forms provided by the EMS agency. The application must be completed in its entirety and shall be verified under penalty of perjury.

Sec. 3-15.7.02. - Review.

The local EMS Agency shall review all applications for license renewal and conduct such investigation as it deems necessary to verify the information pertinent to such renewal application. The investigation shall verify compliance with the requirements as set forth in this Chapter for operation of such services. The local EMS agency shall then prepare a report for the Board as to the request for license renewal, verification of information, and its recommendation of acceptance or rejection of such renewal application.

Sec. 3-15.7.03. - Approval.

If an application for ambulance provider license renewal is approved by the Board, the County shall issue an ambulance provider license, which license shall set forth the class or class of medical services to be provided. Such license shall be valid for a period of no greater than twenty-four (24) months. Nothing in this Chapter, however, shall require the Board to issue such license.
