PGLD/IM inputs a TC 971 AC 505 on the entity portion of an individual's account (as long as the entity is established on the Master File) when all the following occur:
An individual's IRS-held PII was lost, stolen, or disclosed.
The data breach risk assessment results in a likelihood of harm to the potentially impacted individuals.
The IRS notifies the individual of the data breach via Letter 4281C, IM Breach Notification Letter, or similar letter in some circumstances (such as letters developed for the Get Transcript data breach).
