8.120.210 Application form and content.¶
A. Applications shall be in a form prescribed by the administrator and shall contain all of the following information:
The name, mailing address, and daytime telephone number of the person who will be present during, and responsible for, the filming;
If the filming is to be conducted by an organization, the organization’s name, mailing address, and daytime telephone number; the name, address, and telephone number of the corporate agent accepting legal process; and, if requested by the administrator, written documentation of the authority under which the applicant is applying for the permit on behalf of the organization;
The name, mailing address, and daytime and evening telephone number of an alternate person to contact if an emergency arises and the applicant is unavailable;
Name of the film/project;
The proposed date and estimated starting and ending time of the filming;
The proposed filming location, including its boundaries;
The estimated number of cast and crew in filming;
The type and estimated number of vehicles and/or structures that will be used in the filming;
A description of any sound amplification equipment to be employed at the filming;
Anticipated use of any animals, pyrotechnics, fire, or explosives;
Requests for special assistance at the location, including, without limitation, street closure, traffic control, and emergency services; and
Whether the activity to be filmed includes vehicle chases or other activities dangerous to the participants or to the public, with a description of the activity to be filmed.
B. In addition to the provisions of this chapter, any applicant engaging in activity that requires compliance with federal, state, or local regulations, including additional licenses or permits, shall present evidence of satisfactory compliance with those requirements. (Code 1971, § 612120)
8.120.220 Supplemental information required by the administrator.¶
In addition to the information required in this chapter, applications for all permits authorizing filming shall include such supplemental information which the administrator may find reasonably necessary, given the nature of the filming, in order to determine whether to approve or deny a permit authorizing such filming. (Code 1971, § 612130)
