Sec. 4-37. - Application.

No license shall be granted or issued under any of the provisions of this article until the person applying for such license shall file with the treasurer-tax collector of the county, at least thirty days prior to the date such show or exhibition is to take place, a written application, which application shall contain the following information:

The date or dates on which the show or exhibition is to take place, the kind and type of show or exhibition, the number of sideshows and the admission fee, if any, to be charged for each such sideshow, the exact location of the show or exhibition to be licensed and the signature and permanent address of the person applying for same, and a statement that all shows, exhibitions, sideshows and other amusement devices will be conducted in compliance with the laws of the state, the provision of this code and other ordinances of the county.
