Sec. 58-371. - Required.¶
(a) No person shall solicit contributions for any religious purpose within the city without a certificate from the administrative services officer. Application for a certificate will be on forms provided by the city. Such application shall be sworn to or affirmed and shall contain the following information:
(1) The name and address of the person or organization applying for the certificate;
(2) The purpose for which the solicitation is to be made, and the use of any receipts therefrom;
(3) An outline of the methods to be used in conducting the solicitation; and
(4) The time when such solicitations shall be made, giving the preferred dates for the beginning and ending of such solicitation.
(b) Any of the requirements of subsection (a) of this section may be waived by the administrative services officer if the person has, within six months prior to making application to the city, obtained and held an unrevoked certificate or permit from the County or the City of Los Angeles to solicit contributions for religious purposes in either or both of their respective jurisdictions.
(c) If, while any application is pending or during the term of any certificate granted, there is any change in fact, policy, or method that would alter the information given in the application, the applicant shall notify the administrative services officer in writing thereof within 24 hours after such change.

Sec. 58-372. - Issuance.¶
The administrative services officer shall issue the certificate required under this division within ten days of receipt of the information required in section 58-371.

Sec. 58-373. - Investigation.¶
The administrative services officer is authorized to investigate the affairs of any person soliciting for religious purposes under a certificate issued under section 58-372 and the affairs of any person exempted from the requirements of a certificate or a permit under section 58-336 and make public his written findings in order that the public may be fully informed as to the affairs of any of such persons. Such persons shall make available to the administrative services officer or to any representative designated by the administrative services officer in writing for such specific purpose all books, records, or other information reasonably necessary to enable the administrative services officer to fully and fairly inform the public of all facts necessary to a full understanding by the public of the work and methods of operation of such persons. However, five days before the public release of any findings under this section, the administrative services officer must first serve a copy of the findings upon the person investigated, and at the time of the release of his findings he must release a copy of any written statement the person may file with the administrative services officer in explanation, denial, or confirmation of the findings.
