San Fernando Municipal Code Div. 2 Permit

It shall be unlawful for any person to conduct, in or upon any premises within the city, the business of tattooing or body piercing without first obtaining a permit. Except as stated in this division, permits shall be acquired pursuant to and subject to article III of this chapter.

Any application for a permit required pursuant to this division shall contain the following information:

(1) The full name and present address of the applicant.

(2) The address of the site where the proposed business of tattooing or body piercing will be conducted.

(3) Written proof that the applicant is over the age of 18 years.

(4) The applicant's height and weight, and the color of the applicant's eyes and hair.

(5) Two photographs, at least two inches by two inches, showing the full front of the applicant's face only.

(6) A history of past permits or licenses for tattooing or body piercing held by the applicant indicating the identification numbers of such permits or licenses and the issuing agencies. The history shall also indicate whether the applicant has had any such permit or license revoked or suspended, and the reason therefor.

(7) The name, address, and state license number of the licensed physician who will perform or directly supervise the performance of tattooing or body piercing.

Upon receipt of an application for a permit required under this division, the city council shall cause to be investigated the information provided in the application.
