In particular, this announcement corrects certain specifications for checkboxes on Form W–2 and the address to send sample substitute forms to receive approval from the SSA.

Rev. Proc. 2015–55, page 788. Pub. 1167, General Rules and Specifications for Substitute Forms and Schedules, provides guidelines and general requirements for the development, printing, and approval of substitute tax forms.
