INSTRUCTIONS FOR REQUEST TO APPEAR AT SMALL CLAIMS HEARING BY ZOOM Parties requesting to appear by Zoom must follow the instructions outlined below:

1. Submit Required Forms (At Least 10 Days Before the Hearing)
The party requesting a Zoom appearance must obtain and submit the following forms to the Court no
later than 10 days before the scheduled hearing:
o Request to Appear by Zoom
o Declaration Re: Request to Appear by Zoom
o Order Re: Request to Appear by Zoom

2. Complete and Mail Forms
Complete pages 2 and 3, and the top half of page 4. Mail the original forms, along with one set of
copies and a self-addressed stamped envelope, to:

3. Court Review and Decision
After a Judicial Officer reviews the request, the Court will mail a signed copy of the order indicating
whether the request has been granted or denied.
o If the request is granted, proceed to Step 4.
o If the request is denied, an in-person appearance is required for the hearing.
4. Zoom Appearance Information (If Approved)
If the request is granted, the requesting party must appear by Zoom using the information below:

5. Submission of Evidence
If you intend to have the Court consider any documents, photographs, or other evidence at the
hearing, you must provide copies to both the Court and the opposing party(ies) no later than 10 days

6. Day of the Hearing
On the day of your hearing, be prepared to present and discuss your case via Zoom as if you were
appearing in person in court.
