Division 2 — SERVICE APPLICATIONS

7522. • APPLICATION FOR WATER SERVICE.

Each applicant for water service shall be required to provide the following information by telephone, in person, or in writing:

(a)

Name and address of applicant;

(b)

Social Security Number;

(c)

Drivers license number or passport number;

(d)

Address of premises to be served;

(e)

Date on which the applicant will be ready for service;

(f)

Size of service;

(g)

Name and address to which the bills are to be mailed or delivered;

(h)

Whether the applicant is an owner, tenant, lessee or agent for the premises;

(i)

Any such other information as the Department may reasonably request.

The application constitutes a request for service and shall not bind the Department to give service except under reasonable conditions. (Renumbered by Ord. 2143 adopted 8-7-01)
