19.40.600 - General.¶
A mitigation monitoring program shall be prepared for all mitigated negative declarations prior to project approval and for all EIRs as an appendix or attachment to the final EIR.

19.40.610 - Contents.¶
A mitigation monitoring program shall specify:
(a) The party responsible for implementation of each mitigation measure, including costs;
(b) The governmental agency, department or division responsible for monitoring compliance with the mitigation measure;
(c) A time frame for compliance with the mitigation measure; and
(d) Whether a mitigation monitoring report is required to set forth requirements for additional reporting as necessary to ensure compliance with the mitigation monitoring program.

19.40.620 - Basis for revocation of permits.¶
Failure to comply with mitigation measures or monitoring requirements may be grounds for revoking of permits and entitlements for which the environmental document was prepared.
