Sec. 46-116. - Required.¶
Every person who constructs or establishes, causes, permits, or allows to be constructed or established any driveway, entrance or exit connecting a street with private property, wherever curb and sidewalk are maintained, shall first file an application with the director of public works/city engineer in the manner provided in this division and be issued a permit therefor.

Sec. 46-117. - Application contents.¶
Any person or his duly authorized representative who applies for a permit to construct or establish a driveway, entrance or exit shall file an application with the director of public works/city engineer signed and setting forth the following:
(1) The name and address of the applicant.
(2) The name and address of the authorized representative, if any.
(3) Location of the proposed driveway by street number and designating that portion, in feet, to be so used of such lot or property.
(4) The proprietary interest of the applicant in the lot or property.
(5) The overall width of the proposed driveway.

Sec. 46-118. - Deposit.¶
A cash deposit as determined by the director of public works/city engineer shall be paid to the city before a permit is issued under this division, which money shall be held by the director of public works/city engineer, in trust, conditioned upon completing the driveway and the cutting of existing curbs and the finishing thereof according to the specifications and requirements of the director of public works/city engineer. No permit to cut existing curbs or to construct driveways shall be issued until the deposit required in this section has been properly made.

Sec. 46-119. - Fees.¶
The director of public works/city engineer shall charge an appropriate fee as approved by the city council by resolution for the cost of issuing the permit and the cost of inspection of the improvements as provided for by this division.

Sec. 46-120. - Terms.¶
Any permit issued under this division to construct, in any business district, any driveway, entrance or exit shall provide that within 90 days after any use, by reason of which such permit was issued, has been vacated or abandoned, the permittee, owner or lessee shall at his own expense remove such driveway, entrance or exit and restore to its original condition any sidewalk, curb and street so damaged or displaced.
