Most Official Bankruptcy Forms are scheduled to be replaced with substantially
revised, reformatted and renumbered versions effective December 1, 2015. Other National forms (known as Director’s Forms) are being renumbered also effective December 1.
IT IS ORDERED that the following amendments to the Local Bankruptcy Rules
are made as technical corrections to conform with the December 1 National form changes:
2004-1. Examination and Service. (c) The party requesting an examination under FRBP 2004 must use
Local Form CSD 254 2540.
3015-8. Motions to Value Property Subject to Lien Under Plan. (b) Party to Be Named. If a proof of claim has been filed, the party to be
named in the motion is the "Name of Creditor" listed on Form B 10 410.
7003-1. Commencement of Adversary Proceedings. The plaintiff must prepare
and file the completed Form B 104 1040, complaint, and, if needed, Local Form CSD 3007 2500A in accordance with the Administrative Procedures.
7004-1. Service of Process. If a summons is not timely served as provided in
FRBP 7004(e), another summons must be prepared pursuant to Local Form CSD 3007 2500A, with the word "ALIAS" typed above the word "SUMMONS."
7069-3. Registration of Judgments. A judgment by a bankruptcy judge from
any other district may be registered in the Southern District of California by filing with the Clerk a certified copy of such a judgment accompanied by Local Form CSD 3054 2650, and by payment of the Fee mandated by the Judicial Conference pursuant to 28 U.S.C. § 1930.

9013-3. Motions and Applications Not Requiring Notice. (d) Motion Request for extension of time to file certificate of credit counseling under 11 U.S.C. § 109(h)(3) due to exigent circumstance.
9027-1. Commencement of a Removal Action. A notice of removal pursuant to
FRBP 9027 is filed with the Clerk together with: (a) Form B 104 1040 and the complaint.
IT IS FURTHER ORDERED that the period of comment is from now until
November 30, 2015. Except as set forth in further order of this Court, the effective date of this order shall be December 1, 2015. Comments concerning these amendments may be submitted by E-mail to LBRcomments@casb.uscourts.gov [please include the name, firm name (if any), E-mail address, and phone number of the person submitting the comment] or by mail to the Clerk of Court at 325 West F Street, San Diego, California 92101 - 6991. Please note on the envelope: “LBR renumbering”. The deadline is November 30, 2015.
