INSTRUCTIONS:

All paperwork must be typed or printed neatly (legibly) in black ink.

Prepare one (1) original and two (2) copies of all forms for filing with the Clerk’s Office.

Submit the Declaration re: Change of Name (form # MNC-110-local) at the same time the petition is filed.
