Community Service Work Time Sheet Instructions

1. Complete Time Sheet Daily. The Community Service Work Time Sheet must be completed on a daily basis by the individual supervising your work by indicating the date, the number of hours worked and the supervisor’s initials in the space provided. Community service hours can only be performed on or after the date you signed your agreement and paid your community service work fee and only at an agency listed in the Community Service Work / Volunteer Work Agencies provided to you by the court.

2. Out of County Service. If you would like to complete your community service work at an agency outside of Marin County, please ask the court for a list of approved agencies.

3. Get Agency Stamp and Authorized Signature. When you have completed all the hours the time sheet must be imprinted with the agency’s stamp or seal in the space provided. The time sheet must also be signed and dated by an authorized agency representative. The court will not accept the time sheet without the agency stamp or seal and signature.

4. Turn in Your Time Sheet. It is your responsibility to return the completed and signed time sheet to the court. It must be received by the court on or before the due date specified on the time sheet. Failure to return the completed, signed time sheet by the due date will result in your community service work hours being automatically converted back to a fine, due and payable immediately, as described in your community service work agreement.

5. Complete Su Hoja de Asistencia Diariamente. La hoja de asistencia de Servicio Comunitario debe ser llenada diariamente por el individuo que está supervisando su trabajo indicando la fecha, el número de horas trabajadas y las iniciales del supervisor en el espacio provisto. Las horas de Servicio Comunitario solo se pueden llevar a cabo en la fecha o después de la fecha en que usted firmó el acuerdo y pagó el honorario para hacer servicio comunitario y solamente se puede llevar a cabo en una agencia que esté en la lista de Servicio Comunitario o Trabajo Voluntario que el Tribunal le ha dado.

6. Servicio Afuera del Condado. Sí a usted le gustaría completar su Servicio Comunitario en una agencia afuera del Condado de Marin, por favor pídale al Tribunal una lista de agencias aprobadas.

7. Obtenga el Sello de la Agencia y la Firma Autorizada. Cuando halla completado todas las horas, la hoja de asistencia deberá ser estampada con el sello de la agencia en el espacio provisto. La hoja de asistencia también deberá ser firmada y fechada por el representante autorizado de la agencia. El tribunal no aceptará la hoja de asistencia si falta el sello o la firma.

8. Entregue Su Hoja de Asistencia. Es su responsabilidad de devolver la hoja de asistencia completa y firmada al tribunal. El Tribunal deberá recibirla en la fecha o antes de la fecha especificada en la hoja de asistencia. Si usted falla en entregar su hoja llena y firmada en la fecha especificada esto podría resultar en que sus horas de servicio comunitario se conviertan en multa, que se deberá pagar inmediatamente, como se especificó en el acuerdo de trabajo de servicio comunitario.
