Petitioner alleges: 1. Petitioner __________________________________________________________________, is the stepfather/stepmother of the subject minor child ___________________________, and seeks an order freeing the minor from custody and control of the father/mother _______________________________________, and termination of parental rights as to the minor. 2. Petitioner is seeking to adopt _____________________________________________, and a Request for adoption has been filed in the Superior Court of the County of Amador. 3. _____________________________________, is an unmarried minor child and is a resident of ____________________, in Amador County, California. 4.
