(d) Format and Content of Motions and Notices.

(1) Format. All pleadings and documents filed in support and in opposition to
a motion shall contain in the caption the date and time of the hearing and
the courtroom1 in which the hearing will be held. All pleadings and
documents filed in support and in opposition to a motion shall conform
with the Court’s Guidelines for the Preparation of Documents, effective
December 1, 1999, or as thereafter amended.

(2) Separate Notice. Every motion shall be accompanied by a separate notice
of hearing stating the docket control number, the date and time of the
hearing, the location of the courthouse1, the name of the judge hearing the
motion, and the courtroom1 in which the hearing will be held.
