If there has been proper service, and the parties agree to drop or continue the six (6) month or twelve (12) month conference date as set forth below, this form must be submitted. Attach a written agreement or letter confirming the agreement to this form when it is submitted. This form must be submitted to the court for filing at least fifteen (15) court days before the conference date.
