SUPERIOR COURT OF CALIFORNIA - COUNTY OF SAN JOAQUIN Reserved for Clerk’s File Stamp Street Address: Mailing Address: City, State, and Zip:

Case Title:

Case Number: NOTICE OF INITIATION OF ONLINE DISPUTE RESOLUTION (ODR)

PROGRAM – SMALL CLAIMS

The Court provides a free voluntary program to help people resolve their Small Claims case online without a court hearing. This program is called Online Dispute Resolution (ODR) and requires the registration and participation of all parties should they choose to use it.

This is notice that a party in this case has chosen to participate and engage with you through the ODR program.

Should you choose to participate, you will need to register at https://services.sjcourts.org/civilodr/.

After both sides have registered for ODR, parties in a case can communicate and request free help from a mediator who can assist in reaching a settlement agreement. Mediators are provided by mediation agencies, not by the Court. You have until (5) court days before your hearing to complete ODR.

If you reach a settlement agreement, ODR will generate the form with all agreed terms and send it to the Court. If approved, your hearing will be canceled.

If you do not have a court-approved Settlement Agreement by (5) court days before your hearing or do not register because you choose not to participate in this program, you MUST attend your hearing.

Visit the Court’s Small Claims ODR website https://services.sjcourts.org/civilodr/ :
