FORMS PETITION FOR PROBATE GUARDIANSHIP OF THE PERSON Revision Date February 25, 2026 Purpose These forms are used to ask for guardianship of the person. Guardianship of the person is where the court appoints an adult person (not the parent) to have custody, make decisions and care for a minor child. Required Forms You can access the forms by clicking on the highlighted form number below.

• Petition for Appointment of Guardian: GC-210(P) • Child Information Attachment: GC-210(CA)

➢ One for each minor child included in the Petition. • Declaration Under Uniform Child Custody Jurisdiction & Enforcement Act: GC-120 ➢ Attachment to Declaration Under UCCJEA: GC-120(A) (if necessary) • Order Appointing Guardian or Extending Guardianship of Person: GC-240 • Letters of Guardianship: GC-250 • Duties of Guardian: GC-248 • Confidential Guardian Screening Form: GC-212 ➢ One for each Petitioner • Information Release Authorization: VN-043 • Guardianship Questionnaire: VN-042 ➢ If the Petitioners are unmarried, each must fill out a separate form. ➢ A copy of each child’s birth certificate, custody order or other evidence of birth must be attached to this form. • Release of School Information: VN-133 ➢ One for each minor child included in the Petition. • Release of Medical Information: VN-132 ➢ One for each minor child included in the Petition. • Consent of Guardian, Nomination and Waiver of Notice: GC-211 • Notice of Hearing: GC-020 ➢ Attachment to Notice of Hearing Proof of Service by Mail: GC-020(MA) ➢ Proof of Personal Service of Notice of Hearing: GC-020(P)

If requesting a Temporary Guardianship, you will also need the following forms:

(NOTE: Temporary Guardianships are only granted if you can show there is an emergency and an earlier court date is necessary.) • Petition for Appointment of Temporary Guardian of the Person: GC-110(P) • Order Appointing Temporary Guardian: GC-140 • Letters of Temporary Guardianship: GC-150 • Notice of Hearing: GC-020 ➢ Attachment to Notice of Hearing Proof of Service by Mail: GC-020(MA) ➢ Proof of Personal Service of Notice of Hearing: GC-020(P)

Filing Fee There is a filing fee due at filing. To apply for a fee waiver, prepare and file: • Request to Waive Court Fees (Ward or Conservatee): FW-001-GC ➢ One for each minor child included in the Petition. • Order on Court Fee Waiver (Ward or Conservatee): FW-003-GC ➢ One for each minor child included in the Petition.
