Declaration of Address Verification

A new mandatory form, Declaration of Address Verification (WG-015/EJ-135) has been published by the Judicial Council. The levying officer shall not accept or serve any writ, order, notice or other paper on any person, for personal debt, if the judgment creditor fails to provide this form pursuant to CCP 684.130 (2)(3).

Use of Judicial Council Forms

There have been several recent changes to Judicial Council forms. Pursuant to California Rules of Court, Rule 1.37, all Judicial Council forms must be submitted using the current, most recently adopted version. This includes non-mandatory forms. The use of outdated or superseded forms is not compliant. To ensure your documents are served in the timeliest manner, please ensure that you are using the most current Judicial Council forms. This included the SER-001 "Request for Sheriff to Serve Court Papers" and SER-001A "Special Instructions for Writs and Levies," which can be found below under "Letters of Instructions".

Civil Division Mailing Address

Effective immediately, the Placer County Sheriff’s Civil Division is no longer using the Post Office Box at the Auburn Post Office. Please update your records accordingly. Items sent to the previously used Post Office Box may be delayed in being routed to the Civil Division or returned by the Postal Service. ALL by-mail correspondence (documents, instructions, payments/fees etc.) must be mailed to:

The Placer County Sheriff’s Office Civil Division accepts the following forms of payment only;

Certified or Cashier’s Check, Business Check, Money Order, Cash (U.S. Currency).

Services may be mailed, or personally delivered to our office at 2929 Richardson Dr, Auburn CA 95603. Services may be emailed to our office at [email protected]. However, with the exception of most Temporary Restraining Orders, the person requesting service is responsible for printing costs in accordance with California Government Code, Section 26727.

The Sheriff is entitled to his/her fee for service whether the service is successful or not pursuant to Gov § 26720, 26736, 26738, and 26746.
