Packet Fee: $1.75
Filing Fee: $435.00 (Fee Waiver packet available upon request)
No Filing Fee required if the restrained person has used or threatened to use violence against protected person, has stalked the protected person, or has acted or spoken in some other way that makes the protected person reasonably fear violence.
INSTRUCTIONS:
All paperwork must be typed or printed neatly (legibly) in black ink.
Prepare one (1) original and two (2) copies of the forms for filing with the Clerk’s Office.

This packet is to be served on the restrained person along with a copy of the Temporary Restraining Order or, if applicable, a Reissuance.
