  1. The moving party is the stepparent of the following minor child(ren):

  2. There is a pre-existing relationship between the stepparent and the stepchild(ren) that has engendered a bond such that visitation is in the best interest of the child(ren).

  3. Visitation rights ordered under Family Law Code § 3101(c) will not conflict with a right of custody or visitation of a birth parent who is not a party to this proceeding.

  4. To obtain or modify a temporary or permanent visitation order wherein visitation is contested, mediation is required pursuant to Family Law Code § 3170. Mediation is scheduled as indicated in the Notice of Hearing filed simultaneously herewith.

  5. The stepparent shall bear the cost of mediation.

WHEREFORE, the stepparent requests that the Court grant him/her visitation with said minor child(ren), and for such other relief as the court may deem appropriate.
