INSTRUCTIONS: Read this form and initial the box for each item that applies to you, but only if you understand it, and then sign and date the form. If you have any questions about your case, or the information on this form, ask your attorney or the judge.

Misdemeanor or Infraction Trial - I understand I have the right to a trial within 30 days of my 1. arraignment or plea (whichever is later), if I am in custody at the time, or within 45 days of my arraignment or plea, if I am not in custody. If a trial is not held within these time periods, then my case may be dismissed. I give up this right.

2. Felony Trial - I understand I have the right to a trial within 60 days of the day the information 2. is filed. If a trial is not held within this time period, then my case may be dismissed. I give up this right.

3. Preliminary Hearing - I understand I have the right to a preliminary hearing within 10 court 3.

days of the day I enter a plea. I understand I have the right to have my case be dismissed if
the preliminary hearing is not held within 60 days, unless I agree otherwise. I give up my right
to a preliminary hearing within 10 days and within 60 days.
