I hereby request that the Court allow me to submit a Trial by Written Declaration on this case without first having to post the required bail with the Court, as authorized by Vehicle Code section 40902. I declare under penalty of perjury that I am presently financially unable to post the required bail and have filled out the necessary financial qualification form.

I UNDERSTAND THAT BY SIGNING BELOW, I AGREE TO THE FOLLOWING:

I will be assigned a due date upon the Court’s approval of this waiver request. I must return my Trial by Written Declaration by the due date, otherwise I will be found guilty in absentia and sentenced.
