AUTHORIZATION FOR COURT DROP FILE

I hereby authorize the San Luis Obispo Superior Court to maintain a drop file at the Courthouse. (please check either or both branches of the court)

[ ] San Luis Obispo Branch [ ] Paso Robles Branch

I authorize the Court to use my drop file to return conformed copies of filed documents. In addition, I authorize the Court to use my drop file for Court notices in lieu of having them mailed to my place of business.
